В какой форме писать письма в КСС ?

Я впервые собираюсь писать в КСС запрос о предполагаемой дате моего интервью. Честно говоря не очень-то представляю в какой форме писать письмо. Кто знает, пожалуйста проверьте правильно ли я мыслю:



Case Number: xxxxxxxxxxxxx

Name: xxxxxx xxxxxxxxxx

Date of birth: xx/xx/xxxx



Dear Sir/Madam

Could you please let me know what is the status of my case and is there appointment date set for me.



Правильно ли это? Или достаточно просто написать Case number, Name and DOB?

Вот запросы в КСС, как говорится, на все случаи жизни:



 To inquire about your DV 2005 case, on the Subject Line type: Case Number

o In the message text, include complete case number (shown on your Notification Letter) and complete Name and Date of Birth of the principal applicant.



 To request Instructions on Adjustment of Status procedures, on the subject line type: Adjustment



 To request KCC to re-send the Packet of Information that contains the forms you need to complete and return to KCC, on the subject line type: Packet 3

o In the message text, include complete Case Number, Name, and Address. (Note: You will not receive the Notification Letter. See below.)



 For a replacement copy of your Notification Letter: You must submit that request in writing and mail it to:



Kentucky Consular Center

3505 N Hwy 25W

Williamsburg, KY 40769



o Include complete case number

o Provide a recent photo of yourself

o Full Name and Address

o Be sure to sign the request
