Give a brief synopsis of that experience and comment on how the encounter did or did not demonstrate fragmented versus integrated health care. If your chosen encounter demonstrates fragmented care discuss the barriers to care continuity and how the system could be improved. If your chosen encounter demonstrates continuity of care discuss what system factors contributed to this.
